Official version was primary and backup o-ring failure due to launching in too cold weather, combined with wind shear. The o-rings failed immediately at launch, then the hole was plugged with aluminum oxides from the burning propellant. This had happened before, but this time the o-rings were too cold and hard and were damaged worse. They might have made it if they hadn't hit a bad wind shear at about 65 seconds after launch, which shifted everything around and blew the "plug' out, and allowed the SRB to start torching the main fuel tank, and boom. That, and the fact that they were using W style SRB's.
